Getting Started:

All therapy services require a current doctor’s order or prescription for evaluation and/or treatment.

If you are interested in becoming a patient at Riverside Rehabilitation, please contact our office during business hours or discuss your concerns with your doctor.

Appointments:

Therapy appointments are set according to patient and therapist availability and prior commitments / schedules. Most therapy appointments are scheduled 1-3 times weekly for 30-60 minute sessions based on your physician's recommendation and patient availability. Appointments are booked as standing appointments and are usually carried over from week to week.

We make every possible effort to schedule patients at times that are most convenient and do take late appointments as needed.

Intake:

New patients are required to complete a set of intake forms listed to the right. Completing the forms before your visit will speed the intake process.

For your convenience, if we schedule you for an evaluation, we will also provide a case history form. 

On your first visit, you should bring:

•  Any completed forms you may have

•  Your insurance card

•  Driver’s license (or other photo ID)
